The Abbott Nutrition Hospital to Home (H2H) and Paediatric H2H services are designed to help make the move from tube feeding in hospital to tube feeding at home or in the community easier. The service helps to provide you or your child with care and support. It also supplies you or your child with feed and equipment for use at home.

Here are some examples of the sort of support that H2H can offer you or your child:


Training
Before you or your child is discharged from hospital on a tube feed, you will be trained on how to use your feeding equipment through the H2H service.


Managing your feed at home
H2H provides you or your child with a flexible delivery service for your tube feeds and other items. The service offers:


  • Delivery of feeds, giving sets and ancillary items at a time and day convenient for you

  • Delivery straight to your door free of charge; feeds can be delivered by H2H or your local pharmacist

  • Emergency delivery guaranteed within six hours, except Scottish Highlands/Islands and Channel Islands (see below)

  • We can even train school staff on how to use your child’s feeding equipment and deliver feeds to their school so that your child can live as normally as possible.
